Recognized Patient Centered Medical Home by NCQA

Physicians East Internal Medicine Practices in Beulaville, Farmville, Greenville, Grifton, Kinston, and Winterville have received National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experiences of care, and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system.

Research shows that medical homes can lead to higher quality and lower costs and can improve experiences of care.

What is a Patient-Centered Medical Home?
A Patient-Centered Medical Home is a model of health care delivery in which health care professionals work together as a team to provide health care. You, the patient, are the most important part of a Patient-Centered Medical Home. When you take an active role in your health and work closely with us, you can be sure that you’re getting the care you need. Your primary care provider leads your care team.

Your primary care provider focuses on you to help develop health care goals. He or she also is responsible for managing and coordinating your care with other providers, which may include specialists, nurses, and other health care professionals, such as pharmacists or physical therapists.
